Mental Health Disabilities Case Study

Background
GR has been living with mental health disabilities like Schizophrenia, Paranoia with low mood. Hearing voices of a derogatory nature associated with some physical disabilities like cerebrovascular disease, cerebral infarction, hypertension, hypothyroidism and gastric ulcers .
Since 2021, GR have lived with depression, which in the past has been of a severe and acute degree requiring ECT. Her mental health deteriorated, due to which the risk of standing, wandering and risk of falls became very high. She was moved from Woodloes Hospital due to significant history of 22 falls from November 2019 to January 2023.
Intent
Grande Care wanted to undertake a deep dive into GR’s case in order to see what else we could do to reduce falls and prevent potential serious injuries, or loss of mobility that she has suffered from in the past.
Due to her forgetful nature and short memory loss, GR needed to eat and drink more.
Our team’s intention is to make her as independent as possible to enable her to do certain tasks by herself and make her house friendly home environment. In this way we wanted to promote her independence and quality of life as she has always been a house proud lady.
What we did
With the help of the registered manager, senior carer and three other carers who are fully trained, briefed, skilled with additional training on mental health disabilities. We put a number of actions in place to mitigate and reduce the risk of her falling by closely working with her psychologist and occupational therapist. We installed a sensor mat and PIR beam, and regular well-being checks.
We also conducted an analysis for any potential cause, trying to follow the trends or patterns of her falls. We found that she tended to fall more during the early hours of the morning as she got up from bed, and in the afternoon as she forget to use the Zimmer frame while going to the kitchen or to another room.
GR regularly forgot to use the frame, which resulted in the falls. Due to 24 hour 1:1 care being provided and helping by reminding her to use the Zimmer frame all the times while walking in the house followed by the carers. The carers also noticed that she was falling backwards more than falling forwards. We put all the safe measures in place to ensure she is not left alone while manoeuvring around the house or outside.
